Introduction: Sleeve gastrectomy (SG) has been widely disseminated as a surgical treatment for obesity and associated comorbidities, and currently it is one of the most performed surgeries in the world. Experimental research is becoming increasingly relevant to characterize the pathophysiological mechanisms induced by it. Objective: The aim of this study was to standardize an experimental model of SG in rats with obesity induced using a cafeteria diet (CAF) and evaluate variations in weight and glycemic control after vertical SG, maintaining the CAF. Materials and Methods: Twenty Rattus norvegicus albinus rats, Wistar strain, with an average weight of 250 g were used. The animals were randomized into two groups and underwent 4 weeks of obesity induction before the procedure. In 10 animals of the SG group, vertical SG was performed, and in 10 animals of the control/sham (C) group, simulated surgery was performed, consisting of laparotomy and bidigital compression of the stomach. The animals were followed for a total of 8 weeks, with the weight assessed weekly and fasting blood glucose assessed before the start of the CAF, at the time of surgery, and after 4 weeks of the postoperative period, when they were sacrificed. Results: Before obesity induction, the average weight was 257.8 g in the SG group 266.1 g in the C group. After obesity induction, the average weight was 384 g in the vertical sleeve gastrectomy group and 374.8 g in the C group. In the fourth postoperative week, the average weight was 391.6 g in the VSG group and 436.6 g in the C group. The average blood glucose levels were 88.7, 101.8, and 91.3 mg/dL in the VSG group and 86.6, 103.1, and 109.4 mg/dL in the C group, respectively, before the start of the diet, in the fourth preoperative week, and in the fourth postoperative week. Conclusions: Vertical SG in rats is feasible and promotes glycemic control in the postoperative period. CAF allows induction of obesity and changes in blood glucose.

Resumo: Introdução: O cadáver é uma peça insubstituível no aprendizado da anatomia humana, pois permite uma visão tridimensional das estruturas anatômicas. Entretanto, observa-se um cenário de escassez e aumento da demanda das peças cadavéricas nas universidades brasileiras. Logo, diversas instituições, amparadas no artigo 14 da Lei nº 10.406/2002 do Código Civil brasileiro, implementaram programas de doação de corpos interessados em trazer melhorias significativas na qualidade de ensino da disciplina de anatomia. Objetivo: Este estudo teve como objetivo analisar o grau de conhecimento e as perspectivas de uma comunidade universitária a respeito da doação voluntária de corpos cadavéricos para fins educacionais e de pesquisa. Método: Trata-se de um estudo observacional, transversal e prospectivo com abordagem quantitativa e coleta de dados por meio de questionário individual. Os sujeitos incluídos foram os docentes e discentes da Universidade Federal do Maranhão (Ufma), campus Dom Delgado. Os questionários eram relativos à obtenção de informações sobre o conhecimento e a opinião acerca da temática da doação de corpos. Resultado: Obteve-se um total de 264 questionários respondidos, os quais mostraram que 85,65% dos discentes e 87,1% dos docentes tinham conhecimento acerca da possibilidade da doação do próprio corpo. Contudo, observou-se que 94% dos entrevistados não sabiam que procedimentos eram necessários para a doação, e 86% não possuíam conhecimento acerca da legislação que permitia tal prática. Ademais, 27,65% dos participantes estavam aptos a doar o próprio corpo, 33,7% apontaram que não doariam e 38,9% informaram que não tinham opinião formada. O principal motivo apontado para doar foi: "para contribuir com o avanço da educação na área da saúde", e para não doar: "quero ser enterrado ou cremado". Conclusão: Existe um alto percentual de indivíduos que não conhecem os meios para realizar a doação de corpos, sendo evidenciado que os participantes apresentam baixa aptidão para doação. Contudo, existe um percentual significativo de indivíduos que não tem opinião formada sobre o assunto, demonstrando que a divulgação dessa temática é um meio para aumentar a adesão a tal prática. É importante realizar o desenvolvimento de programas de doação de corpos para solucionar a problemática.


Abstract: Introduction: The cadaver is an irreplaceable piece in the learning of human anatomy, allowing a three-dimensional view of anatomical structures. However, there is a scenario of scarcity and increased demand for cadaveric parts in Brazilian universities. Therefore, several institutions, supported by article 14 of Law 10.406/2002 of the Brazilian Civil Code, implemented body donation programs aimed at bringing significant improvements in the teaching quality of the anatomy discipline. Objective: To analyze the level of knowledge and perspectives of a university community regarding the voluntary donation of cadaveric bodies for educational and research purposes. Method: This is an observational, cross-sectional and prospective study with a quantitative approach and data collection through an individual questionnaire. The subjects included in the study were teachers and students from the Federal University of Maranhão, Campus Dom Delgado - UFMA. The questionnaires were related to obtaining information about knowledge and opinions on the topic of body donation. Result: A total of 264 answered questionnaires were obtained, which showed that 85.65% of students and 87.1% of teachers were aware of the possibility of donating their own bodies. However, it was observed that 94% of the interviewees did not know what procedures were necessary for the donation, and 86% did not have knowledge about the legislation that allowed this practice. Furthermore, 27.65% of the participants were able to donate their own body, 33.7% indicated that they would not donate and 38.9% reported that they had no formed opinion. The main reason given for donating was: "to contribute to the advancement of education in the health area", and for not donating: "I want to be buried or cremated". Conclusion: There is a high percentage of individuals who do not know how to carry out the donation of their own bodies, being evidenced that the participants have low aptitude for donation. However, there is a significant percentage of individuals who do not have a formed opinion on the subject, demonstrating that the dissemination of this topic is a means to increase adherence to this practice. It is important to carry out the development of body donation programs to solve the problem.

BACKGROUND: Extended vertical gastrectomy is a variation of the vertical gastrectomy technique requiring studies to elucidate safety in relation to gastroesophageal reflux. AIM: To analyze comparatively vertical gastrectomy (VG) and extended vertical gastrectomy (EVG) in rats with obesity induced by cafeteria diet in relation to the presence of reflux esophagitis, weight loss and macroscopic changes related to the procedures. METHODS: Thirty Wistar rats were randomized into three groups, and after the obesity induction period by means of a 28-day cafeteria diet, underwent a simulated surgery (CG), VG and VGA. The animals were followed up for 28 days in the post-operative period, and after euthanasia, the reflux esophagitis evaluation was histopathologically performed. Weight and macroscopy were the other variables; weight was measured weekly and the macroscopic evaluation was performed during euthanasia. RESULTS: All animals presented some degree of inflammation and the presence of at least one inflammation criterion; however, there was no statistically significant difference in the analysis among the groups. In relation to weight loss, the animals in CG showed a gradual increase during the whole experiment, evolving to super-obesity at the end of the study, while the ones with VG and EVG had weight regain after the first post-operative period; however, a less marked regain compared to CG, both for VG and EVG. CONCLUSION: There is no difference in relation to reflux esophagitis VG and EVG, as well as macroscopic alterations, and both techniques have the ability to control the evolution of weight during postoperative period in relation to CG.

RESUMO Racional: A gastrectomia vertical ampliada é uma variação da técnica da gastrectomia vertical, necessitando de estudos a fim de elucidar a segurança em relação ao refluxo gastroesofágico. Objetivo: Analisar comparativamente gastrectomia vertical (GV) e gastrectomia vertical ampliada (GVA) em ratos com obesidade induzida por dieta cafeteria em relação à presença de esofagite de refluxo, perda de peso e alterações macroscópicas relacionadas aos procedimentos. Método: Trinta ratos Wistar foram randomizados em três grupos, e após período de indução de obesidade por meio de dieta cafeteria de 28 dias, foram submetidos a operação simulada (grupo controle GC), gastrectomia vertical (grupo GV) e gastrectomia vertical ampliada (grupo GVA). Os animais foram acompanhados por 28 dias no pós-operatório e, após a eutanásia, foi realizada a pesquisa de esofagite de refluxo através de avaliação histopatológica. Peso e avaliação macroscópica foram as outras variáveis de estudo, sendo o peso aferido semanalmente e a avaliação macroscópica no momento da eutanásia. Resultados: Todos os animais apresentaram algum grau de inflamação e a presença de ao menos um critério de inflamação, porém, não houve diferença estatisticamente significante na análise entre os grupos. Em relação à perda de peso, os animais do GC apresentaram aumento gradativo durante todo experimento evoluindo para super-obesidade ao término do estudo, enquanto os dos grupos GV e GVA tiveram reganho de peso após a primeira semana do pós-operatório, porém, reganho menos acentuado se comparável ao GC, tanto para GV quanto para GVA. Conclusões: Não há diferença em relação à esofagite de refluxo entre GV e GVA, bem como em relação às alterações macroscópicas. Ambas as técnicas têm capacidade de controlar a evolução do peso no pós-operatório em relação ao grupo controle.

The aim of this study was to identify biochemical changes in sciatic nerve (SN) after crush injury and low-level laser therapy (LLLT) with 660 nm and 808 nm by Raman spectroscopy (RS) analysis. A number of 32 Wistar rats were used, divided into four groups (control 1, control 2, LASER 660 nm, and LASER 808 nm). All animals underwent surgical procedure of the SN and groups control 2, LASER 660 nm, and LASER 808 nm were submitted to SN crush damage (axonotmesis). The LLLT in the groups LASER 660 nm and LASER 808 nm was applied daily for 21 consecutive days (100 mW, 30 s, 133 J/cm2 fluence). The hind paw was removed and the SN was dissected and positioned on an aluminum support to collect dispersive Raman spectra (830 nm excitation, 30 s accumulation). To estimate the biochemical changes in the SN associated with LLLT, the principal component analysis (PCA) was applied. The Raman spectra of the sciatic nerve fragments showed peaks of the major biochemical components of the nerve, especially sphingolipids, phospholipids, glycoproteins, and collagen. The spectral features identified in some of the principal component loading vectors are referred to the biochemical elements present on the SN and were increased in the groups treated with LLLT, mainly lipids (sphingo and phospholipids) and proteins (collagen)-constituents of the myelin sheath. The RS was effective in identifying the biochemical differences in the SN after the crush injury, and LASER 660 nm was more efficient than the LASER 808 nm in cell proliferation and repair of the injured SN.

INTRODUCTION: The caudate lobe is a distinct liver lobe and surgical resection requires expertise and precise anatomic knowledge. Left-sided approach was described for resection of small tumors originated in the Spiegel lobe but now the procedure has been performed even for tumors more than five centimeters. The aim of this study is to present three cases of tumor of caudate lobe underwent isolated lobectomy by left-sided approach. PRESENTATION OF CASE: Three patients with metastasis of colorectal cancer, carcinoma hepatocellular and metastasis of neuroendocrine tumor underwent resection. After modified Makuuchi incision, early control of short hepatic e short portal veins before hepatectomy was performed. The operative time was 200, 270 and 230min respectively. No blood transfusion was used and no postoperative complications were observed. The length of stay was 7, 11 and 5days respectively. DISCUSSION: Some approaches have been described to access and resect tumors of the caudate lobe, including the left-sided approach, right-sided approach, combined left- and right-sided approach and the anterior transhepatic approach. For liver resection in patients with malignant disease, parenchymal preservation is important in order to avoid postoperative liver failure or due to the risk of second hepatectomy. In these patients isolated caudate lobectomy is a safe option. CONCLUSION: Isolated caudate lobectomy is a feasible procedure. Left-sided approach can be preformed even for tumors larger than 5cm.

BACKGROUND: Dunbar syndrome is a rare disorder due to external compression of the celiac trunk by the median arcuate ligament. The symptoms include abdominal pain, nausea and vomiting. Laparoscopic treatment was suggested as a safe procedure. The aim of this study is to present a case of Dunbar syndrome underwent laparoscopic treatment. CASEPRESENTATION: A 17-year-old female patient presented at emergency room with upper abdominal pain and dyspepsia, related to food intake. A selective arteriography of the celiac trunk revealed stenosis due to compression of the celiac artery. The decompression of the celiac trunk by the median arcuate ligament was performed. Postoperative course was uneventful and the patient was discharged on the 5th postoperative CONCLUSIONS: Laparoscopic division of the arcuate ligament in patients with Dunbar syndrome is feasible and safe. This procedure can be performed uneventful in order to restore quality of life of the patient.

PURPOSE: To evaluate the influence of the cyclosporine in liver regeneration in rats submitted to an experimental model of 70% hepatectomy. METHODS: Forty male rats were randomly divided in four subgroups (C.24h, C.7d, E.24h, E.7d), according to the drug used and the day of sacrifice (24 hours and 7 days). Cyclosporine (10 mg/Kg/day) was given to the study subgroup and 1 ml of 0.9% sodium chloride was to the control subgroup. Resection of left lateral lobe and median lobe performing 70% of liver mass. During the animals' death, KWON formula was applied. Counting of mitotic figures and percentage of positive nucleus with PCNA and Ki-67 were evaluated. RESULTS: In the 2nd, 4th PO and death days, E.7d lose more weight than C.7d. Regarding to the KWON formula, the C.7d regenerated more than the C.24h and the same with the E.7d. Comparing between the groups, only E7d subgroup was statistically significant compared with C.7d, showing the stimulating effect of cyclosporine in liver regeneration. Immunohistochemestry had significant results between the study subgroups. The mitotic index revealed statistical differences in the control subgroups. CONCLUSION: Cyclosporine, in spite of being an immunosuppressive drug, has a positive effect in liver regeneration, although reduce the animal's body weight.

OBJECTIVE: To evaluate the influence of sirolimus on liver regeneration triggered by resection of 70% of the liver of adult rats. METHODS: we used 40 Wistar rats randomly divided into two groups (study and control), each group was divided into two equal subgroups according to the day of death (24 hours and seven days). Sirolimus was administered at a dose of 1mg/kg in the study group and the control group was given 1 ml of saline. The solutions were administered daily since three days before hepatectomy till the rats death to removal of the regenerated liver, conducted in 24 hours or 7 days after hepatectomy. Liver regeneration was measured by the KWON formula, by the number of mitotic figures (hematoxylin-eosin staining) and by the immunohistochemical markers PCNA and Ki-67. RESULTS: there was a statistically significant difference between the 24h and the 7d groups. When comparing the study and control groups in the same period, there was a statistically significant variation only for Ki-67, in which there were increased numbers of hepatocytes in cell multiplication in the 7d study group compared with the 7d control group (p = 0.04). CONCLUSION: there was no negative influence of sirolimus in liver regeneration and there was a positive partial effect at immunohistochemistry with Ki-67.

OBJETIVO: avaliar a influência do sirolimo sobre a regeneração hepática desencadeada pela ressecção de 70% do fígado de ratos adultos. MÉTODOS: utilizaram-se 40 ratos Wistar que foram divididos aleatoriamente em dois grupos (estudo e controle), cada grupo foi subdividido em dois subgrupos iguais conforme o dia da morte (24 horas e sete dias). O sirolimo foi administrado na dose de 1mg/kg/dia no grupo de estudo e no grupo controle foi administrado 1ml de solução salina. As soluções foram administradas diariamente, desde três dias precedentes à hepatectomia até a morte dos ratos, para a retirada do fígado regenerado, realizada em 24h ou 7d após a hepatectomia. A análise da regeneração hepática foi mensurada pela fórmula de KWON, número de figuras de mitose pela técnica de hematoxilina-eosina e pelos marcadores imunoistoquímicos PCNA e Ki-67. RESULTADOS: demonstrou-se variação estatisticamente significativa quando comparado os grupos 24h com os grupos 7d através dos métodos de análise. Ao comparar os grupos de estudo e controle no mesmo período demonstrou-se variação estatisticamente significativa apenas pelo Ki-67 no qual foi verificado aumento do número de hepatócitos em multiplicação celular no grupo de estudo de 7d quando comparado com o grupo controle de 7d (p=0,04). . CONCLUSÃO: não demonstramos influência negativa do sirolimo na regeneração hepática e houve efeito parcial positivo pela análise imunoistoquímica utilizando Ki-67. .

OBJETIVO: Avaliar em dois momentos distintos da regeneração hepática a influência do Tacrolimus sobre o fenômeno da regeneração hepática desencadeada pela ressecção de 70 por cento do parênquima hepático em ratos plenamente desenvolvidos. MÉTODOS: Utilizaram-se 40 ratos Wistar com peso médio de 510,08 g ± 11.66 g distribuidos aleatoriamente em dois grupos de 20, cada grupo subdividido em dois subgrupos conforme o dia da morte após a hepatectomia. De acordo com o grupo os animais receberam por gavagem solução aquosa de Tacrolimus 0,1 mg/kg/dia ou solução salina no mesmo volume. Após três dias de pré-terapia todos foram submetidos à hepatectomia de 70 por cento pela ressecção dos lobos hepáticos mediano e lateral esquerdo que foram pesados para posterior cálculo da regeneração hepática pela fórmula de Kwon. Vinte e quatro horas ou sete dias após a hepatectomia, 10 animais de cada grupo foram mortos, os fígados remanescentes (regenerados) foram pesados e amostrados para realização de índice mitótico por hematoxilina-eosina e estudo imunoistoquímico com os marcadores PCNA e Ki-67. RESULTADOS: Os animais que receberam tacrolimus mostraram índice maior de regeneração hepática, atingindo significância estatística quando comparado ao subgrupo de animais mantidos com placebo quando analisados pelos parâmetros: fórmula de Kwon, índice mitótico e marcador PCNA. A tendência para o marcador Ki-67 foi idêntica aos outros parâmetros mas não alcançou significância estatística. CONCLUSÃO: A imunossupressão com tacrolimus possui efeito estimulatório no processo de regeneração hepática desencadeado pela hepatectomia 70 por cento em ratos Wistar adultos, plenamente desenvolvidos.


OBJECTIVE: To analyze, during two different moments of liver regeneration, the effect of the immunosuppressant Tacrolimus on the 70 percent hepatectomy model-induced liver regeneration in adult rats. METHODS: Forty Wistar adult rats, weighing 510.08 + 11.66 g were randomly divided into two groups, each group divided into two subgroups according to the death day after 70 percent hepatectomy . According to the group of study, rats received 0.1mg/Kg/day of Tacrolimus or the same volume of saline solution, by gavage. After three days of pre-therapy, all animals were submitted to 70 percent hepatectomy by resection of median and left lateral hepatic lobes which were weighed for posterior calculation of liver regeneration by Kwon´s formula. Twenty four hours or seven days after hepatectomy ten rats of each group were killed; the remaining liver (regenerated) was entirely resected, weighed and sampled for mitotic index on hematoxylin-eosin staining and immunohistochemical assays with PCNA and Ki-67 markers. Data were statistically analyzed by Mann-Whitney or Student "t" tests, with significance level of 5 percent (p<0.05). RESULTS: Rats receiving tacrolimus showed statistically significant higher levels of liver regeneration when compared to placebo according to Kwon's formula, mitotic index and PCNA marker. Identical trend was found with Ki-67 marker, but without statistical significance. CONCLUSION: Tacrolimus-based immunossuppression has stimulatory effect on liver regeneration process induced by 70 percent hepatectomy in adult Wistar rats.

RESUMO

RACIONAL: A regeneração hepática representa o resultado de respostas fisiológicas que ocorrem após perdas de parênquima, como consequência de agressões tóxico-medicamentosas, infecciosas, traumáticas ou cirúrgicas. OBJETIVO: Avaliar, em dois momentos distintos da regeneração hepática, a influência do tacrolimus sobre o fenômeno da regeneração hepática desencadeada pela ressecção de 70


do parênquima hepático em ratos plenamente desenvolvidos. MÉTODO: Utilizaram-se 40 ratos Wistar com peso médio de 510,08 g ± 11.66 g divididos aleatoriamente em dois grupos de 20 (Estudo e Controle), cada grupo subdividido em dois subgrupos conforme o dia da morte após a hepatectomia (24 horas e 7 dias). De acordo com o grupo os animais receberam por gavagem, diariamente, iniciando três dias antes da hepatectomia e prolongando-se por todo o período de observação, solução aquosa de tacrolimus 0,1 mg/kg/dia ou solução salina no mesmo volume. OBJECTIVE: To determine the opinion and knowledge of the population of Curitiba about organ donation and transplantation. METHODS: The opinion and knowledge about organ donation and transplantation of 1,000 subjects over 18 years of age were determined. The subjects responded to a questionnaire of 20 queries. Respondents had age, gender, social-economic, and education distributions similar to those of the Brazilian population, as defined by the Brazilian Institute of Geography and Statistics (IBGE). RESULTS: Eighty-seven percent of respondents were in favor of organ donation. There was no difference in the percentage of respondents in favor of donation in relation to gender, marital status, religion, and income. The main reasons in favor of donation were to save life, to help other persons and to donate life. The main reasons against donation were distrust towards medicine or the Brazilian transplantation organization, the existence of organ sale, and fear of body mutilation. Most respondents believed that wealthy people have a better chance to receive an organ than poor people, that sales of organs exists in Brazil, and that misdiagnosis of brain death may occur. CONCLUSION: Most respondents are in favor of organ donation and have a good knowledge of organ donation and transplantation. The majority distrusts Brazilian organization of organ distribution and brain death diagnosis.

RESUMO

OBJETIVOS: Determinar a opinião e o conhecimento, de uma amostra da população da cidade de Curitiba, sobre doação e transplante de órgãos. MÉTODOS: A opinião e o conhecimento sobre doação e transplante de órgãos de mil pessoas, com idade superior a 18 anos, foram determinados através de um questionário de 20 perguntas. Os entrevistados avaliados tinham uma distribuição similar em idade, sexo e padrão socioeconômico, e escolaridade à população brasileira, determinada pelo IBGE. RESULTADOS: A maioria dos entrevistados (87,8 por cento) era favorável à doação de órgãos. Não houve diferença na percentagem da população favorável à doação de órgãos com a relação a sexo, estado civil, religião e rendimentos. Os principais motivos para serem favoráveis à doação de órgãos foram para salvar vida, ajudar o próximo e doar vida. Os principais motivos para não serem favoráveis à doação foram falta de confiança na medicina ou no sistema de captação e distribuição de órgãos, por haver comércio de órgãos e temor de mutilação do corpo. A maioria dos entrevistados respondeu que os ricos tinham mais chances de serem transplantados do que os pobres, que existe venda de órgãos no Brasil e que existe possibilidade de erro no diagnóstico de morte encefálica estabelecido pelo médico. CONCLUSÃO: A maioria da população deste estudo é favorável à doação de órgãos e tem um bom conhecimento sobre doação e transplante de órgãos. A maioria demonstrou não confiar no sistema de distribuição de órgãos e no diagnóstico de morte encefálica.
